RN Care Manager — Community Home Health, Full-Time

Community Care, Inc.

Community Care is seeking an RN Care/Case Manager in Mount Pleasant, Wisconsin. This full-time role supports our aging and disabled population with coordinated care, home-based services, and timely access to appointments. The position offers an 8:00 a.m. - 4:30 p.m. schedule, eligibility for a $8,000 sign-on bonus, and a comprehensive benefits package.
