Nurse Care Manager

Role Description The Nurse Care Manager is a full-time, hybrid role based in Baltimore, MD, with a mix of on-site and work-from-home responsibilities. This role coordinates patient care across settings, including assessment, care planning, implementation, and evaluation to support optimal health outcomes. Day-to-day tasks include conducting clinical assessments, monitoring patient progress, providing education and counseling, and working closely with physicians, social workers, and other team members to ensure continuity of care. The Nurse Care Manager facilitates transitions of care, helps address barriers to treatment adherence, and advocates for patient needs and preferences. The role also involves documenting care plans and outcomes in clinical systems, participating in quality improvement initiatives, and following UMB and regulatory standards. Qualifications Clinical nursing expertise, including patient assessment, care planning, and care coordination skills. Strong communication, counseling, and interpersonal skills to collaborate effectively with patients, families, and interdisciplinary teams. Organizational and time-management abilities for managing caseloads, prioritizing tasks, and meeting documentation requirements. Comfort with electronic health records (EHR) and basic computer skills for data entry, reporting, and telehealth or remote communications. Knowledge of care management principles, population health, and community resources to support comprehensive patient care. Current Registered Nurse (RN) license in Maryland (or eligibility for licensure) and graduation from an accredited nursing program; BSN preferred. Experience in ambulatory care, chronic disease management, case management, or a similar setting is beneficial.
