Requirements Must have TX license, DEA and CDS to request app Full Privilege Requirements Need to have completed hospital application, COI, all required credentials, verification of all education/training/licensure/boards, claims history of past 5 years, all current/prior (10 years) work history and affiliations verified, three peer references received, NPDB, background check Physician Requirements Summary Active State Medical License, CDS, & DEA (with TX address), Board Certified in ABEM OR ABFM OR ABIM (or osteopathic counterparts), *Must be residency trained in Speciality Certified in ACLS, ATLS, & PALS, CME past 2 years, case logs past 2 years, copies of all education & training diplomas, SS Card, Driver's license, color photo emailed to MSO, TB/FLU, Annually must have 9 hrs of Stroke CME and 9 hrs of Trauma CME. Current ATLS not required for ABEM providers & only need to provide copy of initial ATLS.Facility Coverage 24 hours Physician Coverage Shift Times 7a-7p, 7p-7a Procedures Required Yes Procedures Type Consistent with EM procedures Average Patient Volume by Provider 34 patients per day Hospital Privileging Requirements Privileging Timeframe: 90-120 days
