Hospice Certified Nurse Assistant/Home Health AideEmployment Type: Full timeLocation: Enfield RegionStatus: Full TimeBonus: Sign On BonusTrinity Health Of New England At Home serves patients and their loved ones by providing a variety of in-home services including but not limited to nursing, social work and physical/occupational/speech therapies.We are committed to providing compassionate, exceptional care where people are most comfortable: at home!A Catholic, Mission-driven, non-profit organization, we are affiliated with Johnson Memorial Hospital of the Trinity Health Of New England health system and are part of Trinity Health At Home, a National Health Ministry of Trinity Health, one of the largest Catholic healthcare systems in the United States.Position Purpose:Our home health aides provide paraprofessional services to our patients in their homes, following physician orders and in compliance with the state's Nurse Practice Act, any applicable licensure/certification requirements and our policies and procedures.Position Details:Schedule: Monday - Friday, 8 am - 5 pm, one weekend every 4 weeks and two rotating holidays per year.Coverage Region: EnfieldBonus: Position qualifies for a $5,000 sign-on bonus paid out over two years.NOTE: This position is 100% home care providing services in patients' private homes.What You Will Do:Follows care plan developed by RN.Reports client changes in condition to RN, as described on care plan.Assists clients with personal care activities including bathing, mouth care, denture care, skin care, grooming, bathroom privileges, transferring, ambulation, and dressing and undressing activities.Assists clients with household tasks including, planning and preparing meals, feeding, light housekeeping, changing linens, making bed, and laundering clothes and bedding.Measures and records client's oral, rectal and axillary temperatures, pulse respiration and blood pressure as ordered.Performs range of motion and other simple procedures as an extension of therapy services as assigned.Assists clients in self-administration of medication. This procedure must be assigned by an RN and documented in the clinical record.
