Pay Range: 2349.20 - 2466.66

Schedule: 08:00 - 16:00

Contract Length: 13 weeks

Candidate Type: Traveler

Requirements: RN with 2+ years recent and relevant home care experience, OASIS and Epic experience required

Responsibilities:

• Conduct home care visits, including start of care and revisits
• Manage a caseload of 25-30 visits per week
• Assess patient needs and develop individualized care plans
• Document visits accurately in OASIS and Epic systems
• Coordinate with healthcare teams to ensure quality care
