Harmony Hospice Care is seeking a dependable, compassionate, and organized RN Case Manager to join our team.

This is an opportunity for a nurse who believes in the heart of hospice care — building meaningful relationships, supporting patients and families, and providing care with professionalism, consistency, and compassion.

Compensation and Benefits

Salary: $85,000 annually Benefits: Telehealth, with dental and vision options available

The Role

Manage a structured caseload of up to 18 patients

Serve patients in San Antonio and surrounding areas just outside city limits

Build strong, supportive relationships with patients and families

Collaborate with a team that values communication, accountability, and compassion

Who We’re Looking For

We are looking for an RN who is:

Compassionate and patient-centered

Reliable and accountable

Organized and able to manage responsibilities independently

Comfortable with travel throughout the assigned territory

Committed to providing thoughtful, consistent hospice care
