We are seeking an experienced Hospice Registered Nurse (Primary RN / Case Manager) for a 13-week travel assignment.


Grand Junction, Colorado
**Start Date:** July 13, 2026
⏳ **Contract Length:** 13 Weeks
**Shift:** Monday – Friday | 8:00 AM – 5:00 PM
⏰ **40 Hours Per Week**

**Requirements:**
✔ Active Registered Nurse (RN) License
✔ Minimum 3 Years of Hospice Nursing Experience
✔ Strong Case Management and Care Coordination Skills
✔ Excellent Assessment and Symptom Management Experience
✔ Compassionate, Patient-Centered Approach to End-of-Life Care


**Preferred Qualifications:**
✔ Previous Community Hospice Experience
✔ Strong Interdisciplinary Team Collaboration
✔ Experience with Patient and Family Education
✔ Excellent Communication and Documentation Skills


**Responsibilities:**
• Manage a dedicated caseload of hospice patients in community settings
• Coordinate individualized plans of care with the interdisciplinary team
• Perform comprehensive patient assessments and ongoing evaluations
• Provide symptom management and end-of-life nursing care
• Educate patients and caregivers on disease progression and comfort measures
• Coordinate services across homes, assisted living facilities, and nursing facilities
• Maintain accurate and timely clinical documentation
• Participate in rotating on-call and occasional weekend coverage as needed
• Deliver compassionate, high-quality hospice care while supporting patients and families
