Primary RN - Case Manager
Location - Grand Junction, CO
Term/Duration- 13 weeks
Scheduled hours - 40 hrs
Shift- Mon-Fri 8am-5pm
Responsibilities:
- Assess patients and develop individualized plans of care.
- Manage symptoms and provide skilled nursing care based on patient needs.
- Educate patients, families, and caregivers on disease progression, medications, and caregiving techniques.
- Lead and collaborate with the interdisciplinary hospice team.
- Coordinate care across home, facility, and community settings.
- Maintain accurate and timely clinical documentation.
- Participate in occasional weekend and on-call backup coverage.
Requirements
- Current Registered Nurse (RN) license.
- Minimum 2 years of experience required.
- Hospice or home health nursing experience preferred.
- BLS
