Responsibilities The Physical Therapist primarily provides outpatient rehabilitation services in an in‑home setting for older adults, including patient residences and independent living facilities. The role emphasizes evaluating, developing, and delivering therapy programs tailored to the patient’s environment and needs, referring to other disciplines, and instructing patients and caregivers to promote independence. Implement individualized therapy programs to restore, reinforce, and enhance performance. Instruct, educate, and train patients and caregivers in essential skills. Direct activities to promote and maintain health. Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel per state licensure requirements. Ensure adherence to treatment schedules, documentation, billing, and related meetings. Attend and contribute to patient care, staffing conferences, and other meetings. Make referrals and consult with other team members to select appropriate therapeutic regimens. Adapt programs and plans of care based on patient needs. Promote a safe environment and enforce fire and safety hazard elimination. Order, maintain, and report equipment needs. Prioritize patient service and provide compassionate care. Screen, examine, and evaluate patients; establish diagnosis and treatment goals. Design and implement therapeutic techniques, including exercise, manual therapy, airway clearance, electrotherapeutic modalities, and health education. Integrate health and wellness programming into treatment plans. Perform home and community mobility assessments and recommend safe mobility solutions. Perform routine reexaminations and adjust treatment plans as needed. Carry out other related duties as required. Qualifications Valid Physical Therapy license in the state(s) of practice or proof of license eligibility. Master’s degree in Physical Therapy or a related field with a Bachelor’s in Physical Therapy; alternatively, Bachelor’s in Physical Therapy plus five years of progressively responsible experience. Degrees earned outside the U.S. must be evaluated as equivalent to a U.S. Physical Therapy degree. Valid driver’s license or reliable transportation to patients’ residences.
