Develops the plan of care for each patient, in coordination with the primary care provider, and oversees that plan of care for effectiveness.Assesses patient’s condition at each home visit. Documents that assessment accurately and reports to the physician as needed.Provides direct patient care and demonstrates knowledge in home care/hospice as well as scientific and technical principles, drugs, and therapeutics. Carries out physician orders, dressing changes, venipuncture, home infusion, medication reconciliation, etc. as specified in the plan of care.Performs and maintains documentation.Follows all HIPAA guidelines and promotes and ensures patient safety.Fulfill mission statement by performing job duties with a high level of customer service while contributing to a positive and professional environment.Special projects and other duties as assigned.Education and/or ExperienceGraduate of an accredited school of nursing. Previous Home Health & Hospice experience preferred. Electronic Health Records experience and management experience preferred.Licensure/CertificationCurrent Registered Nurse licensure in the State of Nebraska.Basic Life Support (CPR)Advanced Cardiac Life Support (ACLS) preferredValid Driver’s LicensePhysical DemandsThe physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.Ability to operate a motor vehicle and complete errands based on business needs.Work EnvironmentWhile performing the duties of this job, the employee regularly works in a medical clinic setting and patient residences.
