Physical Therapist - Travel Contract

Physical Therapist — Home Health, Demopolis, AL

Seeking a traveling Physical Therapist to provide patient evaluations and therapeutic services according to physicians‘ orders in a home health setting. The clinician will perform treatment, assist physicians in evaluating function, develop and update plans of treatment, document and report patient condition, instruct and consult with family and agency personnel, evaluate the home environment, and participate in in-service education. The therapist will directly supervise and evaluate Physical Therapy Assistants and Home Health Aides as assigned. Reliable transportation, current driver’s license, and agency-required liability insurance are required for home visits.

Minimum Qualifications

• Successful completion of a physical therapy program accredited by the APTA, the Council on Medical Education and Hospitals of the AMA, or the appropriate accrediting body.
• Current, unencumbered license to practice physical therapy in the state of assignment prior to the start of the assignment.
• Current BLS/CPR certification from AHA or ARC.
• Experience in home health or providing in-home therapeutic services preferred.
• Familiarity with electronic documentation, email, internet navigation, and basic computer functions required.
• Ability to supervise and evaluate PTAs and home health aides as assigned.
• Reliable transportation, current driver’s license, and agency-required liability insurance required for home visits.
• Two to three years of physical therapy experience preferred.

Preferred Qualifications

• Previous home health agency experience with supervision of PTAs.
• Experience coordinating discharge plans and performing on-site assessments and reassessments.

Assignment Details

• Evenings, 5x8
• Home visits required. Must have reliable transportation and current driver’s license.
