Overview
Physical Therapist – Outpatient, Mattoon, Illinois. Start date August 3, 2026. 13-week travel assignment in a hospital setting. Weekly pay range $2,195 to $2,246 per week. Guaranteed hours 40 per week. Shift: 5x8 Days (8:00 AM–4:30 PM).

Responsibilities
- Plan and execute rehabilitation programs to help patients return to a higher level of physical function, using therapeutic exercise, neuromuscular re-education, modalities, gait training, and manual techniques.
- Perform initial and ongoing physical therapy evaluations; develop and adjust a Plan of Care with short- and long-term goals, in collaboration with other staff as needed.
- Document patient progress according to approved medical and facility documentation procedures; develop discharge plans with home exercise programs and caregiver instructions.
- Evaluate mobility, strength, and current function; implement the Plan of Care on a consistent schedule and modify as patient progress dictates.
- Maintain open communication with therapy staff, department colleagues, and referral sources; supervise physical therapy assistants as appropriate.
- Maintain patient records and ensure compliance with certifications, authorizations, and third-party payer requirements; pursue ongoing professional development through workshops and literature.

Requirements
- Active Illinois Physical Therapist license
- Active American Heart Association BLS certification
- 1+ year physical therapy experience
- Outpatient experience
- Candidate residency: permanent address outside a 50-mile radius of the assignment location (non-local candidates only)
- Documentation: 2 Supervisory Reference Forms with dates of employment; Skills Checklist current within the last year
