- Conduct evaluations for patients in both inpatient and outpatient environments and create tailored treatment plans
- Deliver physical therapy interventions that prioritize mobility, strength, endurance, and overall function
- Collaborate with nurses, physicians, case managers, and other rehabilitation professionals to maximize patient outcomes
- Provide education to patients and families about therapeutic objectives, discharge processes, and at-home exercises
- Maintain comprehensive documentation of evaluations, treatments, and progress in accordance with facility protocols and industry regulations
- Participate in care team rounds and interdisciplinary meetings to promote integrated patient care
- Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution
- State licensure required for Physical Therapists
- CPR certification is preferred
- Additional specialized certifications (e.g., neurological, geriatric) are advantageous but not mandatory
- Previous experience is strongly recommended
- Competence in addressing patients with intricate medical and mobility challenges
- Excellent communication and teamwork capabilities
- Proficient in utilizing digital charting and clinical documentation software
