Description

• Conduct thorough evaluations of patients in both inpatient and outpatient environments and develop tailored treatment plans.
• Deliver physical therapy interventions targeting mobility, strength enhancement, balance improvement, and functional restoration.
• Work in partnership with nurses, physicians, case managers, and rehabilitation experts to ensure optimum patient outcomes.
• Educate patients and their families about therapeutic objectives, strategies for discharge, and effective home exercise routines.
• Keep accurate records of evaluations, treatments, and patient progress in compliance with both facility and regulatory standards.
• Engage in care team discussions and interdisciplinary meetings to facilitate a seamless continuum of care.

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from a recognized institution.

Certifications

• Active state licensure as a Physical Therapist
• CPR certification is highly preferred.
• Specialized certifications (e.g., in neurological or geriatric therapy) are advantageous but not mandatory.
