Care Manager LCSW - Psychiatric 4E

Description Care Manager LCSW at Providence Little Company of Mary Medical Center San Pedro in San Pedro, CA. This position is full-time, 8‑hour day shifts. Care Management within the Department of Psychiatry involves collaborating with patients, caregivers, nurses, social workers, physicians, payers, staff, other practitioners and the community. Social workers facilitate communication and care coordination along a continuum through effective transitional care management, supporting the patient’s right to self‑determination, and addressing social determinants of health and complex care barriers such as coping difficulties, abuse, mental health, substance use, lack of resources and psychosocial issues. Required Qualifications Master's Degree in Social Work. All LCSWs must complete coursework/training required by the Los Angeles County Department of Mental Health for LPS designation and maintain current status. Upon hire: California Clinical Social Worker License. Within 18 months of hire: Designated as Behavioral Health Clinician to perform 5150 evaluations. Within 1 year of hire: CPI certification (non‑violent crisis intervention training). Two years of healthcare‑related experience with a psychiatric/mental health patient population, preferably in an inpatient setting. Computer experience. Preferred Qualifications Discharge planning experience.
