In this hands-on, street-level role, you'll work alongside law enforcement to engage vulnerable individuals, build trust, and deliver immediate care and essential services in the community. You’ll be part of a high-impact team focused on outreach, crisis support, and resource navigation, all while promoting dignity, self-determination, and long-term stability for individuals experiencing homelessness, mental health challenges, and substance use issues. What You’ll Do: Conduct street outreach and attend community events to engage with at-risk individuals Provide emergency support, referrals, and transportation assistance Perform intake assessments and create individualized service plans Mentor clients and assist with accessing basic needs and ongoing care Maintain real-time documentation and ensure continuous support Facilitate group sessions and collaborate with community resources What You Need: High school diploma or equivalent 2+ years of experience working with homeless or behavioral health populations Compassion, strong communication skills, and the ability to problem-solve on the spot A valid driver's license and ability to meet agency driving standards Bilingual is a plus!
