Description

• Conduct thorough evaluations of patients in both inpatient and outpatient settings and formulate personalized treatment strategies
• Implement a variety of physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function
• Work collaboratively alongside nurses, physicians, case managers, and other rehabilitation professionals to maximize patient outcomes
• Guide patients and their families through therapeutic goals, discharge planning, and necessary home exercises
• Accurately document patient evaluations, treatments, and progress in line with facility and regulatory requirements
• Engage in care team discussions and interdisciplinary meetings to promote cohesive care delivery

Education

• Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification preferred
• Additional specialized certifications (such as neurological or geriatric certifications) are a bonus but not essential

Skills

• Prior experience is highly desirable
