Ambulatory Clinical Pharmacist - Refill Center

Job Overview This job monitors and evaluates drug therapy for appropriateness based on the patient’s health status, institutional criteria/guidelines, and cost‑efficient measures. It assumes responsibility for the management of pharmacy services of assigned areas to improve patient outcomes, provides drug information to health‑care personnel, participates with the appropriate medical team on patient‑care rounds, precepts pharmacy students and residents, and provides in‑services/education to pharmacy and other health‑care providers when appropriate. This position will handle refill requests for Primary Care, expediting the refill process, saving provider time, and promoting patient safety. It also supports providers by completing E‑Consults for topics related to drug information, dose adjustments, and order navigation. Education and Training Required: Completion of a PGY‑1 Pharmacy Practice Residency (3 years of related specialty experience may be accepted in lieu of PGY‑1 Residency) Preferred: Doctor of Pharmacy (PharmD) Preferred: Completion of a PGY‑2 Specialty Pharmacy Residency Work Experience Preferred: Ambulatory experience Certifications Required: License to practice Pharmacy by the LA Board of Pharmacy Knowledge, Skills, and Abilities Must have computer skills and dexterity required for data entry and retrieval of patient information; proficient with Windows‑style applications and keyboard. Effective verbal and written communication skills and the ability to present information clearly and professionally to varying levels of individuals throughout the patient‑care process. Strong leadership and teaching skills. Job Duties Provides pharmacokinetic consultation, obtains and uses appropriate laboratory data to evaluate drug medical selection and dosing, and communicates findings to health‑care practitioners. Evaluates patient response to drug therapy. Documents interventions and conducts outcome studies/evaluations related to clinical activities performed, thereby exhibiting good patient outcomes. Assists with the development of pathways, best practices, and guidelines for assigned populations. Educates staff, students, patients, health‑care providers, and other health‑care professionals; submits results for publication once every two years. Mentors residents, students, and new staff to meet departmental goals. Is responsible for overall patient care beyond the hospital visit, through all transitions of care. Volunteers to perform additional work or tasks during staffing shortages caused by inclement weather, electrical power outages, computer downtime, or other situations. Adapts behavior to the specific patient population, including respect for privacy, method of introduction, adapting explanation of services or procedures, requesting permissions, and communication style. Physical and Environmental Demands Frequently requires exerting 10 to 20 pounds of force to move objects; occasionally requires up to 100 pounds of force.
