Hospice NP: End-of-Life Care Leader
Affinity Health Management
Affinity Health Management seeks a Nurse Practitioner to deliver comprehensive hospice services under physician supervision in home-based settings. You will evaluate health histories, diagnose conditions, and direct care plans while coordinating with families and the hospice team. You will perform physical assessments, manage medications, determine hospice eligibility, and contribute to quality care in alignment with state Nurse Practice Act.
