Hospice Nurse Practitioner — End-of-Life Care Leader

One-Community-Hospice is seeking a Nurse Practitioner to provide comprehensive hospice services under physician supervision. You will evaluate health history, diagnose conditions, prescribe treatments, and direct the plan of care within the hospice team and patient homes. The role requires NP certification, state license, and experience in hospice or palliative care.
