Details Work in the Case Management department of a hospital setting Care team includes RN Case Managers and Social Worker Case Managers Patient ratio is at least 25:1 for case management EMR systems used are Meditech and Midas Day shift schedule from 8:30 AM to 5:00 PM Required to work every other weekend and holidays in rotation with hospital employees May float to other units as needed Requirements Active FL or Compact RN license required Current BLS certification required (AHA or ARC accepted, must not expire within first 30 days of start) Minimum 1 year of recent case management experience required Meditech experience required Midas experience required Complex discharge planning experience preferred No gaps in work history equaling 90 days or more within the past 12 months US Social Security Number and date of birth required for consideration Two professional references required for Florida candidates Additional Information Responsible for case management duties including complex discharge planning Must reside outside a 50-mile radius of the facility, as local candidates are not accepted Navy blue scrub color required for RN staff First-time travelers may be considered for this role American Traveler Job ID P-709157.
