Job Details

• Locations:
  • 1. Cumberland County (Mechanicsburg / Camp Hill, PA)
    2. Dauphin (Hershey / Lower Paxton, PA)
    3. Lancaster County (Elizabethtown / Lexington / Eden, PA)
• Weekly Estimated Pay: $3,465 - $3,850
• Shift: 8-hour days, 5 shifts per week
• Duration: 13 weeks
• Start Date: ASAP

Job Requirements

• Graduation from an accredited health professions school with active national board certification as a Nurse Practitioner or Physician Assistant
• Active licensure in the state of Pennsylvania
• Active DEA licensure and state-specific controlled substance registration, as applicable
• Certification in Basic Life Support (BLS)
• Ability to travel as needed with proof of liability and property damage insurance on vehicle used
• Minimum of 1 year of experience in a relevant healthcare field

Responsibilities

• Deliver flexible clinical care both in-person at patients' homes and virtually
• Provide compassionate, individualized, and holistic patient care, including comprehensive primary care, urgent care, and chronic condition management
• Collaborate with a multidisciplinary care team for transitions of care support and complex care management
• Engage in professional development opportunities and collaborate with experts in geriatric medicine and palliative care
• Participate in after-hours on-call coverage as needed
