What You'll Do:

• Evaluate, plan, and implement individualized physical therapy programs for long-term care patients
• Administer therapeutic procedures including manual techniques, ambulation, exercises, modalities, and wound care
• Assess patient progress, modify treatment plans, and document outcomes accurately
• Support discharge planning and provide counseling to patients, families, and caregivers
• Communicate effectively with healthcare team members regarding patient status and treatment adjustments
• Ensure all documentation complies with regulatory standards and facility policies
• Supervise and support Physical Therapist Assistants, rehabilitation aides, or students as needed

What You Need:

• Valid Physical Therapist license for Oklahoma
• Minimum 1 year clinical experience, preferably in LTC or similar settings
• Strong clinical skills and ability to work independently and in team environments
• Effective communication and documentation skills

Preferred:

• Experience with wound care and therapeutic modalities
