Pay Range: $30/hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

Key Responsibilities:

\n

\n

\n
• Conduct monthly and quarterly member assessments, collaborating with caregivers and providers to gather essential information for care plan development.
\n
• Monitor service delivery and follow up with members through in-person visits and telephonic contact.
\n
• Authorize and coordinate referrals for services, ensuring continuity of care without gaps.
\n
• Assist members with filing and resolving complaints and appeals, advocating for their needs.
\n
• Collaborate with discharge planners, physicians, and other stakeholders to develop comprehensive care plans.
\n

\n

Key Requirements and Technology Experience:

\n

\n

\n
• 2+ years of Care Management experience, specifically in a field setting.
\n
• Experience managing caseloads of 50-70 members, preferably in geriatric long-term care.
\n
• Familiarity with Medicaid and Medicare processes.
\n
• Proven ability to work in a fast-paced environment and adapt to new processes and programs.
\n
• Valid Driver’s License is required.
\n
• Experience with electronic medical health records and home health services.
\n
• Experience in discharge planning.
\n
• Familiarity with TruCare software.
\n
• Strong technology skills and time management abilities.
\n
