Responsibilities

• Clinical Care – The Physician Assistant (PA) or Nurse Practitioner (NP) provides assessment, triage, and therapeutic management of patient and family needs throughout the disease process as an integral member of an interdisciplinary team. The NP/PA works in collaboration with the attending supervising/collaborating physician(s) in accordance with state law, hospital regulations, policy, protocols, and clinical pathways. Responsibilities include obtaining medical histories, performing physical examinations, ordering and reviewing diagnostic and therapeutic tests, and formulating and managing patient treatment plans. The NP/PA may perform procedures and prescribe treatments and medications in accordance with institutional and state guidelines.
• Patient Education & Psychosocial Support – The NP/PA educates patients and families on care plans and recommended resources, including admission instructions, treatment plans, potential side effects, and discharge planning. The NP/PA provides psychosocial support to patients, family members, and significant others in a caring, ethical, and professional manner.
• Information Management – The NP/PA ensures appropriate documentation in accordance with hospital and regulatory requirements and protects patient information in all forms from unauthorized access, modification, destruction, or disclosure.
• Quality Management – The NP/PA demonstrates high standards of moral and ethical behavior, professionalism, and compassion, and ensures compliance with patient‑related healthcare regulatory requirements, standards, and recommendations.
• Professional Development – The NP/PA continuously engages in activities that maintain and advance professional competency, serves as a clinical resource for new house staff, and may provide service‑specific orientation related to clinical education. The NP/PA may serve as a clinical preceptor for NP/PA students and other healthcare professionals, collaborate on research, quality assurance, and performance improvement initiatives, and assist in developing protocols that support clinical practice.

Qualifications

• Education – Graduate from an accredited Nurse Practitioner or an accredited Physician Assistant Program.
• Experience – Clinical working experience as an NP/PA in the specialty/service line is preferred but not required. APRNs, PAs, CRNAs, and CNMs must be appointed to the Affiliated Medical Staff of a YNHHS hospital and go through the Medical Staff credentialing process. Evidence of current competence to practice as an APRN, PA, CRNA, or CNM must be provided.
• Licensure – Active PA Certification by NCCPA, Connecticut State Physician Assistant License, ACLS/BLS Certified, and DEA License when indicated. Pediatric position profile requires PALS and NRP certification. Additional licensure, certifications, or continuing education credits may be required based on position and specialty. OR Connecticut State Registered Nurse License, Connecticut State Nurse Practitioner License, Board specialty certification as appropriate.

Smilow Cancer Hospital – Shoreline Medical Center – Guilford provides access to expert cancer care and treatment to patients and families along the Connecticut shoreline and surrounding communities. Our board‑certified doctors, physician assistants, nurse practitioners, nurses, and support staff treat many different types of cancer, including rare conditions. Chemotherapy is available on‑site at all of our offices, with concurrent radiation therapy available at most locations. We work to make your journey back to health as simple as it can be.
