Overview Position: APP PALLIATIVE CARE (DAYS) Location: Smilow Network Scheduled Hours: 7 :30am to 6:00pm Position Type: Full-Time EMR System: Epic What Makes This Opportunity Unique?
- Smilow s erves as the clinical facility for the esteemed Yale Cancer Center, an NCI-designated Comprehensive Cancer Center, the only NCI-designated site in Connecticut.
- The Smilow enterprise includes the Cancer Hospital in New Haven and 16 ambulatory locations across the states of Connecticut and Rhode Island .
- Smilow Cancer Hospital treats more"patients than any other hospital in Connecticut and is QOPI accredited by the American Society of Clinical Oncology.
- Smilow Cancer Hospital is on the cutting edge of personalized medicine, offering a comprehensive approach to patient care with the latest scientific advances in treatment and clinical trials
- I nstitutional Member of Advanced Practitioner Society for Hematology & Oncology (APSHO)
- Advance Practice Provider Clinical Ladder
- Provide assessment, triage, and therapeutic management of patient/family needs throughout the disease process as an integral part of an interdisciplinary team.
- W ork in collaboration with the attending supervising/collaborating physician(s) in accordance with state law and hospital regulations, hospital policy, protocols , and clinical pathways, and may provide autonomous team-based care.
- Obtains medical histories, performs physical examinations, reviews/orders diagnostic/therapeutic tests and radiologic results, chemotherapy prescribing, formulates and manages patient treatment plans.
- The APP will educate patients and families on care and available resources, in collaboration with the interdisciplinary team . This includes admitting instructions, treatment plans, potential side effects, and discharge plans.
- The APP , as part of a inter disciplinary team, provides psychosocial support to patients, their family members, and significant others in a caring, ethical, and professional manner.
- Graduate from an accredited Nurse Practitioner or an accredited Physician Assistant Program.
- 1-2 Oncology and Palliative Care experience is preferred
- Active, unrestricted CT medical license or in process of obtaining
- ACLS/BLS (as required ) Certification
- DEA License
