What You'll Do

• Provide and document nursing care to patients in their homes
• Collaborate with healthcare providers and community resources to coordinate care
• Evaluate patient responses to interventions and adjust care plans accordingly
• Promote, maintain, and restore patients' health through skilled nursing interventions
• Exercise independent judgment while following established protocols and guidelines
• Maintain accurate records and communicate clinical data clearly
• Be flexible with work hours and willing to accept on-call responsibilities

What You Need

Required:

• Valid MA LVN or LPN license
• Ability to drive and work independently in varied weather conditions
• Strong critical thinking and interpersonal skills
• Willingness to accept on-call duties and occasional flexible hours

Preferred:

• Prior experience in home health nursing
• Knowledge of patient care standards in a home setting
