Pediatric Nurse PractitionerHot JobPediatrics Campus - Santa Rosa, CA 95407; Parkway Site - Santa Rosa, CA 95407OverviewSalary Range $138,831.00 - $175,666.00 Salary Position Type Full-Time/Part-TimeDescriptionPOSITION TITLE: Pediatric Nurse PractitionerREPORTS TO (TITLE): Medical DirectorSALARY RANGE: $138,831 - $175,666 DOEJob Summary: As a member of the Patient Centered Medical Home Team Pediatric Nurse Practitioner, under the clinical supervision of the Medical Director, assists in creating partnerships between individual patients seeking care at the Santa Rosa Community Health and the providers by delivering excellent, comprehensive primary health care, prevention, and health education.Duties and ResponsibilitiesCreates an ongoing relationship with each patient by taking complete medical history, performing physical exams and making assessments and diagnoses on teen and pediatric patients; initiating, reviewing, and revising treatment and therapy plans, including developmental assessments, preventative well teen and child care, family planning care, and the promotion of childhood vaccinesMaintains adequate documentation of all patients seen, including follow-up and referral in the electronic medical record.Manages the stable phases of chronic illnesses in consultation with the attending physician or Medical Director as needed.Orders and interprets laboratory reports and diagnostic imaging reportAssesses mental health and provides for treatment as needed in individuals and families.Coordinates health management with physicians and other agencies.Assures quality and safety by a care planning process, evidence-based medicine, and active participation of patients in decision-making.Recognizes emerging community health problems and helps initiate appropriate interventions through community action.Meets with the Medical Director or Attending Physician on a regular basis to review charting and medical care provided.Seeks consultation on any difficult patient as described in the mid-level protocols in the Medical Administrative protocols Manual of SRCH.Participates in ongoing training and quality improvement measuresQualificationsQUALIFICATIONS:EducationCurrent unrestricted California license as a Pediatric Nurse PractitionerExperienceAt least two years experience in a community health center setting preferredAbility to work independently with physician telephone backupKnowledge and SkillsCurrent CPR certificationFluency in Spanish and English (speak, read, and write) preferredDemonstrates knowledge of compliance issues within the community clinic environmentDemonstrates a willingness to report any incident that is unusual or incompatible with accepted clinic procedures.Complies with HIPAA policiesStrong interpersonal skills with the ability to be sensitive with persons of various social, cultural, economic, and educational backgroundsProficiency with Microsoft Office applications including Outlook, Word, Excel and PowerPointStrong organizational skills with ability to prioritize projects, work relatively independently, manage multiple tasks, and meet deadlinesStrong written and verbal communication skillsAbility to work independently and as part of a teamGood judgment, problem solving and decision-making skillsSRCH is an equal opportunity employer to all, regardless of age, ancestry, color, disability (mental and physical), exercising the right to family care and medical leave, gender, gender expression, gender identity, genetic information, marital status, medical condition, military or veteran status, national origin, political affiliation, race, religious creed, sex (includes pregnancy, childbirth, breastfeeding, and related medical conditions), and sexual orientation.Physical Requirements :Use standard office equipment and access, input, and retrieve information from a computer.
