At SNAHC, it is not typical for an individual to be hired at or near the top of the range for their role and compensation decisions are dependent on the facts and circumstances of each case. A reasonable estimate of the current range is $65,384.31-$88,461.13. Position Summary The Community Health Program Manager is an integral position to the operations of the Community Health Department within SNAHC. It is responsible for overall management of programs and activities targeted to families and adults, and will also be responsible for resource management, including the management of grants that fund services. The Program Manager has management and responsibility over grant objectives and deliverables, which include programming, evaluation, and budget/financial management. The Grant Manager will continue to strengthen community participation in program design, evaluation, and policy advocacy. The program manager will work closely with other department managers to ensure programs and activities are aligned with broader department and organizational goals focused on health equity. This role will have an assigned primary location but may be required to work at or travel to different locations as needed. Essential Functions Responsible for the managing the implementation and evaluation of federal and non-federal grant programs focused on the American Indian and Alaska Native community and target populations. Responsible for oversight and completion of grant projects and deliverables, as well as reporting requirements. Responsible for the supervision and staff development of grant project coordinators who implement grant deliverables. Works with project evaluator(s) regarding epidemiological, evaluation and outcome factors. Works with evaluator to develop assessment and evaluation tools in target communities to benefit grant activities. Assures documents and promotional material of a professional standard that comply with branding policy, suitable for internal/ external communication target audiences. Establishes, expands, facilitates and sustains initiatives, programs, and/or partnerships that engage key stakeholders and community members (e.g. local tribes, American Indian education programs etc.). Supports SNAHC leadership in cross-Department processes and systems, including the development of organization initiatives and policies and procedures. Ensures staff and stakeholders are trained in a culturally and linguistically sensitive family- and youth-friendly/driven manner. Acts as an advocate and liaison between community members and the larger system of care to ensure community needs are articulated and heard. Represents SNAHC at the federal, state and county levels. Ensures direct reports’ adherence to department and agency policies, procedures and protocols, including HIPAA compliance and maintenance of confidentiality. Performs management duties including recruiting, onboarding, timekeeping, performance evaluations, coaching and progressive discipline as needed, using SNAHC prescribed best practices. Actively participates in and guides internal quality improvement teams. Works with members proactively to drive quality improvement initiatives in accordance with the mission and strategic goals of the organization, federal and state laws and regulations, and accreditation standards. At all times demonstrates cooperative behavior with supervisors, subordinates, colleagues, clients and the community. Complies with all state and federal laws and regulations, as they pertain to the position including; HIPAA, sexual harassment, Scope of Practice, OSHA, SCAR reporting, etc. Other duties as assigned Skills And Abilities Excellent verbal and written communication skills. Must have excellent organizational and computer skills including experience with word processing and spreadsheet skills. Familiarity with Client-Driven Care. Competency in implementing community engagement strategies. Specific knowledge of youth prevention strategies in alcohol and other drug prevention, tobacco use, risk reduction, and healthy behaviors, expected. Experience working in youth and family advocacy services.
