Physical Therapist (PT) – Home Health

Location: Indiana (Washington, Scott, Partial Floyd, and Partial Clark Counties)

Schedule: Monday–Friday, 8:00 AM–5:00 PM
On-Call Requirement: Minimum of one weekend per month
Productivity Requirement: 30 points per week
Mileage Reimbursement: $0.46 per mile

Position Overview

We are seeking a compassionate and motivated Physical Therapist (PT) to join our Home Health team serving patients throughout Washington, Scott, portions of Floyd, and portions of Clark Counties in Indiana. This position offers the opportunity to work independently while helping patients improve mobility, regain function, and achieve their highest level of independence in the comfort of their homes.

The ideal candidate will have home health experience, strong clinical assessment skills, and proficiency with Homecare Homebase (HCHB) and OASIS documentation.

Key Responsibilities

• Perform comprehensive physical therapy evaluations based on physician orders and referrals.
• Review patient medical records, diagnoses, precautions, contraindications, and treatment history.
• Develop individualized treatment plans with measurable goals focused on maximizing patient independence and functional outcomes.
• Provide skilled physical therapy interventions in accordance with established care plans.
• Regularly assess patient progress and modify treatment plans as necessary to achieve optimal outcomes.
• Communicate patient status, progress, and recommendations to physicians, interdisciplinary team members, patients, and family members.
• Consult with physicians and healthcare providers regarding treatment plans, patient progress, and discharge recommendations.
• Recommend, obtain, and train patients on adaptive equipment to improve safety, mobility, and function.
• Complete all clinical, treatment, and billing documentation accurately and within required timeframes.
• Maintain compliance with company policies, HIPAA regulations, professional standards, and applicable state and federal laws.
• Supervise Physical Therapist Assistants (PTAs) and Physical Therapy Aides in accordance with Indiana state practice regulations.
• Participate in staff meetings, continuing education opportunities, and professional development activities to maintain and enhance clinical skills.

Qualifications

• Current, unrestricted Physical Therapist (PT) license in Indiana.
• Graduate of an accredited Physical Therapy program.
• Home Health experience required.
• Experience with Homecare Homebase (HCHB) required.
• OASIS experience required.
• Strong knowledge of rehabilitation principles, treatment planning, and patient assessment.
• Excellent communication, organization, and documentation skills.
• Ability to work independently while collaborating effectively with an interdisciplinary team.
• Valid driver's license, reliable transportation, and willingness to travel throughout the assigned territory.
