RN Case Manager – Case ManagementSchedule: Full-Time; Weekend Coverage: 5 hours for one Saturday each month; take 5 hours off on one day of the week.Assignment Details: Contract Weeks: 8; Guaranteed Hours: 36Requirements:Bachelor's Degree in Nursing (Master's Degree preferred)Current unrestricted Registered Nurse license in the stateAt least three (3) years of clinical experience in a hospital settingAt least five (5) years of case management experience in a hospital settingCertification in case management (preferred) or willingness to obtain within two (2) years of employmentRoles & Responsibilities:Assess, plan, implement, coordinate, monitor, and evaluate options and services to meet clients' health and human service needs.Maintain and enhance the case management system through ongoing interaction with patients, physicians, and healthcare providers.Document case management plans clearly and concisely.Collaborate with interdisciplinary teams to facilitate effective and cost-efficient patient care.Educate staff and physicians on appropriate levels of care and utilization issues.Stay current with case management practices through ongoing education and participation in professional activities.Client Details:City: Grand JunctionState: COTLC Nursing Associates, Inc. is an equal-opportunity employer and fully complies with all applicable federal, state, and local laws.
