We are a compassionate and growing home health care agency seeking a Licensed Physical Therapist to provide quality, one-on-one therapy services to patients in their homes. This is your chance to work independently while being part of a supportive and experienced team that values excellence in care.

Location: Greater Grand Rapids area

Schedule: Per diem

Responsibilities:

• Provide skilled physical therapy services to patients in their homes

• Create individualized care plans based on functional goals

• Communicate with physicians, patients, and caregivers to ensure coordinated care

• Document visits and progress using EMR systems

Requirements:

• Current State PT License

• At least 1 year of clinical PT experience preferred

• Home health experience a plus (but we provide training!)

• CPR BLS

• Valid driver’s license and reliable transportation
