Description

• Assess and evaluate patients in both inpatient and outpatient settings, creating customized treatment plans
• Deliver physical therapy interventions that focus on enhancing mobility, improving strength, and increasing balance and overall function
• Work alongside nurses, physicians, case managers, and interdisciplinary professionals to maximize patient outcomes
• Educate patients and their families about therapeutic goals and strategies for discharge planning and home exercises
• Keep detailed documentation of evaluations, treatments, and patient progress in line with facility policy and regulatory requirements
• Engage in care team discussions and interdisciplinary meetings to support seamless healthcare delivery

Education

• Earned a Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• Current state licensure as a Physical Therapist
• CPR certification preferred
• Additional specialized certifications (such as neurological or geriatric) are advantageous but not mandatory

Skills

• Previous experience is highly preferred
