Description

• Assess patients in both inpatient and outpatient environments, creating personalized treatment plans
• Deliver physical therapy interventions aimed at enhancing mobility, strength, balance, and overall functionality
• Collaborate with a multidisciplinary team including nurses, physicians, case managers, and rehabilitation professionals to ensure optimal patient outcomes
• Educate patients and their families regarding therapeutic objectives, discharge planning, and home exercise protocols
• Maintain comprehensive documentation of evaluations, treatments, and patient progress in line with facility and regulatory standards
• Engage in care team discussions and interdisciplinary conferences to promote seamless care

Education

• Doctorate in Physical Therapy (DPT) or a comparable degree from a recognized institution

Certifications

• State licensure as a Doctor of Physical Therapy
• CPR certification is preferred
• Specialized certifications (such as neurological or geriatric) are advantageous, though not mandatory
