Homebase Medical is expanding rapidly into the Burbank, CA area. Responsibilities Conduct Annual Wellness Visits focused on patient education. Deliver Chronic Care Management to improve long‑term outcomes. Perform comprehensive in‑home visits including medication review & chronic illness management, medical history updates & physical exams, labs, diagnostic screenings, and post‑hospital follow‑ups. Provide primary care management & palliative care services. Seek out and incorporate technology such as AI to improve practice and member experience. Qualifications Active Nurse Practitioner or Physician Assistant license in the State of California. Board certification (Nurse Practitioner or Physician Assistant). DEA licensure. Prior experience preferred, but passion and commitment are equally important. Ability to work independently in patients’ homes and to collaborate with a team of providers.
