As an IRIS Consultant (IC) , you'll partner with older adults and individuals living with disabilities to develop personalized plans that support their goals, independence, and quality of life. This role combines relationship-building, advocacy, problem-solving, and community engagement to empower participants through Wisconsin's IRIS (Include, Respect, I Self-Direct) program. In addition to supporting participants, you'll have the opportunity to contribute to a collective leadership team that values trust, collaboration, innovation, and shared decision-making. What You'll Do Build trusting relationships with IRIS participants and those important in their lives. Support participants in understanding and maximizing IRIS services and resources. Develop and maintain person-centered IRIS Support and Services Plans. Assist participants in obtaining and maintaining funding for long-term care supports. Connect individuals to resources that promote independence, inclusion, and community engagement. Maintain accurate documentation and ensure compliance with program requirements. Participate in peer mentoring, team collaboration, hiring activities, and ongoing learning. Strengthen community connections and support networks for those served. May conduct Long-Term Care Functional Screens. What We're Looking For Qualified candidates must meet one of the following: Bachelor's degree in Social Work, Psychology, Human Services, Counseling, Nursing, Special Education, or a related field and at least one year of supervised experience working with older adults and/or individuals with disabilities; OR Four years of direct experience providing social services to older adults and/or individuals with disabilities in community-based settings; OR Current IRIS Consultant in good standing with a Wisconsin Department of Health Services exemption. Additional Qualifications: Passion for person-centered services and self-directed care. Ability to work independently and collaboratively. Strong communication, relationship-building, and problem-solving skills. Knowledge of community resources and human services. Proficiency using technology including smartphones, tablets, and computer systems. Strong organizational and documentation skills.
