Housing Case Manager Job Location: South Park - Tucson, AZ 85714 Position Type: Full Time Salary Range: $18.30 - $22.42 Job Category: Nonprofit - Social Services Scope of Work The Housing Case Manager provides comprehensive case management and housing support services to chronically homeless persons, chronically homeless families, and persons living with HIV/AIDS who receive housing through SAAF’s Housing Programs. The role supports participants and their households in obtaining and maintaining housing and ensures access to medical care and supportive services. It is a full‑time non‑exempt position housed at our Park Avenue location and reports to the Housing Case Manager Coordinator. Functional Responsibilities Provide a range of client‑centered services to support chronically homeless persons, families, and persons with HIV/AIDS in obtaining or maintaining housing. Initial assessment of service needs, including the Self‑Sufficiency Matrix. Complete the Vulnerability Index – Service Prioritization Decision Assistance Tool (VI‑SPDAT) with clients assessing for housing. Develop a comprehensive, individualized service plan. Coordinate the services required to implement the plan. Monitor clients to assess the efficacy of the plan. Periodically re‑evaluate and adapt the plan as necessary. Advocate for clients and review utilization of services. Assist clients with obtaining employment and other sources of earned income. Complete the housing application process for potential participants, assessing service needs with the Self‑Sufficiency Matrix. Collaborate with participants to develop a housing service plan that addresses transition and long‑term stability needs. Support participants in meeting identified needs. Actively participate in weekly housing meetings and coordinate priority lists and move‑in activities with SAAF Property Management. Conduct home visits to assess and support clients, identify unmet needs, and make institutional visits as applicable. Attend and participate in treatment team meetings with medical providers, networking with other agencies and community service providers to secure appropriate, quality services for clients. Monitor medication adherence and treatment compliance. Ensure participants understand tenancy agreements and SAAF client rights and responsibilities. Maintain timely and accurate documentation of all client contact. Serve as a member of the Care Services Department, working closely with case management and housing teams. Attend agency staff meetings. Comply with SAAF employment and quality assurance policies and procedures. Qualifications Minimum Qualifications Bachelor’s degree in social services, education, health, or a related field, or one year of direct client/participant volunteer experience in a human service setting. Reliable transportation and a valid Arizona driver’s license. Valid insurance and a driving record supported by SSAF’s liability insurance provider. Proficiency with Microsoft Office and database systems. Effective oral and written communication skills. Strong organizational skills and problem‑solving ability. Ability to work independently. Appropriate fingerprint clearance through the Arizona Department of Public Safety (paid for by SSAF). Preferred Qualifications Knowledge of HIV/AIDS issues. Two years of case management experience working with people with substance abuse and/or mental health issues. Knowledge of supportive housing programs and issues related to homelessness. Experience working with Tucson human service providers. Excellent interpersonal skills and sensitivity to cultural and personal diversity. Working Conditions/Physical Demands Operate in a professional office environment and require the ability to sit for long periods with intermittent walking, standing, bending, squatting, pulling, and pushing. Use standard office equipment such as scanners, computers, keyboards, phones, shredders, photocopiers, and filing cabinets. Occasional travel between SAAF campuses and other locations is required. Physical Requirements This job is a sedentary job, as defined by the Social Security Administration, CFR. Sedentary work involves lifting no more than 10 pounds at a time and occasionally lifting or carrying small items such as docket files, ledgers, and tools.
