• Experience: Strong outpatient primary care experience preferred

What You’ll Do

• Provide comprehensive primary care for adult patients in an outpatient setting
• Evaluate, diagnose, and treat both acute and chronic conditions
• Manage care for post-discharge patients requiring transitional care
• Offer primary care to patients referred from specialty clinics
• Conduct preventive health screenings and chronic disease management
• Coordinate care with specialists and interdisciplinary teams
• Document patient encounters accurately within Epic EMR
• Participate in limited phone/beeper call coverage
• Support quality improvement initiatives aimed at enhancing patient outcomes
• Deliver compassionate, culturally competent care to diverse, underserved populations

What You Need

• Board Certification in Family Medicine or Internal Medicine
• Strong outpatient primary care experience with adult patients
• Ability to manage complex medical conditions effectively
• Excellent clinical judgment and diagnostic skills
• Strong communication and patient education skills
• Proficiency with electronic medical records, preferably Epic
• Ability to work collaboratively in a multidisciplinary team
• Organizational and time-management skills
