Compensation

• Salary: $78,000 – $88,000 annually

Schedule & Workload

• Full-time home health visits
• Minimum productivity: 6 points per week
• Manageable patient territory
• On-call rotation: 1 in 5

Responsibilities

• Perform skilled nursing visits in patients’ homes
• Conduct patient assessments and develop care plans
• Administer medications and treatments
• Provide wound care and patient education
• Coordinate care with physicians and interdisciplinary team
• Complete accurate and timely documentation
• Monitor patient progress and adjust care as needed

Qualifications

• Active Registered Nurse (RN) license
• Home Health experience preferred (not required)
• Valid driver’s license and reliable transportation
• Strong assessment and documentation skills
• Ability to work independently and manage time effectively
• Excellent communication and patient care skills
