Salisbury, North Carolina, United States About the job Nurse Practitioner Salisbury, NC Job Description Responsibilities: Collaborate with the interdisciplinary team to assess, diagnose, and formulate treatment plans for residents with acute or exacerbating medical conditions. Conduct comprehensive physical assessments and review medical histories to determine the appropriate course of treatment for residents. Order and interpret diagnostic tests, including laboratory work, imaging studies, and other relevant tests to aid in diagnosis and treatment planning. Provide timely and evidence-based medical interventions, including medication administration, wound care, IV therapy, and other treatments as needed. Manage and monitor residents in critical or unstable conditions, ensuring appropriate interventions are taken promptly. Work closely with attending physicians, consulting specialists, and other healthcare professionals to ensure seamless coordination of care and communication. Educate residents and their families about treatment options, disease management, and preventive care strategies. Document all assessments, interventions, and outcomes accurately and promptly in the electronic medical records system. Participate in regular multidisciplinary team meetings to discuss resident progress, care plans, and any necessary adjustments to treatment strategies. Contribute to quality improvement initiatives by actively participating in performance improvement projects and promoting evidence-based practice. Provide guidance and mentorship to nursing staff on best practices for acute care management, fostering a culture of learning and professional growth. Qualifications: Master's or Doctorate degree in Nursing with a specialization in Acute Care Nurse Practitioner. Current state Advanced Practice Registered Nurse (APRN) license and Acute Care Nurse Practitioner certification. Proven experience as an Acute Care Nurse Practitioner, preferably in a long-term care or nursing home setting. Strong clinical assessment and diagnostic skills, with the ability to make informed decisions in high-pressure situations. Excellent communication skills to effectively interact with residents, families, colleagues, and other healthcare professionals. Proficiency in using electronic medical records (EMR) systems for documentation and communication. Ability to work collaboratively in a team-based healthcare environment.
