What You'll Do As a Physical Medicine & Rehabilitation Nurse Practitioner, you will: Provide comprehensive medical care to geriatric and rehabilitation patients within the SNF setting Evaluate and manage patients' rehabilitation and physical medicine needs Develop and monitor individualized treatment plans Collaborate closely with nursing, physical therapy, occupational therapy, speech therapy, administration, and families Identify and address medical issues before they become emergencies Participate in preventative care and quality-improvement initiatives Help reduce avoidable hospitalizations and improve patient outcomes Communicate effectively with patients, families, and the interdisciplinary care team Serve as a clinical resource and trusted partner to the facility Practice with a high degree of autonomy while having support from the KMG team Who We're Looking For: We're looking for an NP who is clinically confident, compassionate, collaborative, and genuinely enjoys working with older adults.
