Provide direct therapy, including therapeutic exercise, gait training, balance work, mobility training, strength, and functional activities. Implement evidence-based treatment techniques and modalities appropriate for older adults in a SNF setting. Reassess progress, document progress notes, and adjust plans of care to meet patient goals. Collaborate with physicians, nursing staff, and therapy assistants; communicate changes in patient status. Ensure timely documentation and adherence to facility protocols and regulatory requirements.
