Pivot at Sorrento – San Antonio, TX 78229 Position Type: Full Time Job Category: Clinical Support Cover: Sorrento and Windemere – San Antonio Overview The overall purpose of the Restorative Aide position is to support in the provision of restorative and/or rehabilitative care of ProCare I-SNP members as appropriate. Responsibilities Provide personal care to patients in a manner conducive to their safety and comfort. Assist in the provision of restorative or rehabilitative care. Perform restorative nursing tasks such as ambulation, range of motion, positioning, self-feeding, etc., within the scope of individuals' training. Promote and support the greatest possible degree of independence for patients. Advise the Director of Rehab (DOR) of patient status daily. Contribute knowledge of patient conditions/needs to patient care plans. Assist in the implementation of care plans as directed within the parameters of established policy and procedure. Observe and report on patient conditions and document observations as required. Qualifications State certification as a Nursing Assistant is mandatory. Ability to read, write, and speak the English language. Must be available to work six days per week. Ability to travel to additional SNF locations within the geographical area.
