Position Summary The Home Health Registered Nurse delivers high-quality, specialized clinical care to patients in their homes. Reporting to the Clinical Nurse Manager, this role assesses patient needs, creates and updates plans of care, and coordinates with interdisciplinary teams (PT, OT, ST) and physicians to ensure seamless, compliant care. The RN may also supervise and coach nursing staff to maintain safety and performance standards. Key Responsibilities Conduct initial assessments, re-evaluations, and manage customized plans of care. Provide specialized nursing procedures, clinical counseling, and patient/family education. Coordinate care with doctors, therapists, and supervisors; update medical records promptly. Supervise junior nursing staff, ensure regulatory compliance, and participate in quality improvement. Qualifications & Requirements Active Arizona RN License. 2-3 years clinical RN experience (at least 1+ years in home health or related care). Wound care and IV certifications preferred. Ability to travel daily, work flexible shifts (nights/weekends/holidays), and lift up to 50 lbs. #J-18808-Ljbffr Place 2B Properties
