What You'll Do:

• Engage and collaborate with Referral Partners to secure all required information and documentation.
• Leverage all available resources to help our customers understand and navigate the home healthcare referral process.
• Provide assistance to customers telephonically, electronically, or by other organization-approved means.
• Receive and document referral information and medical orders.
• Assign diagnosis codes and process referrals in a timely manner to appropriate service line for admission.
• Collect and document information about the patient's situation and functional capacity, relative to the care plan post discharge.
• Maintain patient records in accordance with established protocols and policies.
• Friday-Monday Schedule, four 10-hour shifts.

What You'll Bring:

• Active RN license in the State of Maine.
• 2 years recent RN experience.
• Basic computer knowledge and data entry experience.

Hours: 9:00am - 5:30pm. Thursday- Monday. Must participate in a holiday rotation.
