Case Manager Per Diem

Case Manager – Per Diem – Las Cruces, New Mexico Coordinating patient care for inpatient rehabilitation patients, collaborating with physicians, therapists, and other interdisciplinary team members to develop and maintain care plans. Ensures patients progress toward desired outcomes, identifies and resolves barriers, and actively participates in discharge planning. Integrates the hospital’s mission and guiding principles into daily practice. Qualifications Required Skills Current clinical licensure strongly preferred. One year of recent relevant experience required, acute care setting preferred. Additional Qualifications/Skills Understanding of treatment costs and financial support related to quality and efficiency. Critical thinking skills: using logic and reasoning to identify strengths and weaknesses of alternative solutions and approaches. Leadership skills in delegating, organizing, and educating coworkers and staff. Knowledge of community resources to meet post-discharge clinical and social needs. Knowledge of clinical operations and procedures. Ability to establish and maintain proactive relationships with interdisciplinary team members, payers, physicians, community organizations, and health care facilities. Ability to maintain quality, safety, and infection control standards. General computer skills: data entry, word processing, email, record management. Effective organizational and time management skills. Effective written and verbal communication skills. Ability to maintain confidentiality. Ability to work closely and professionally with others.
