Clinical Case Manager - Homeless Outreach Specialist
The People Concern
The People Concern's Santa Monica HMST is seeking a master’s level clinical case manager for field-based services with chronically homeless individuals facing co-occurring mental health and/or medical issues. You will work on an interdisciplinary team, develop individualized service plans, help clients enroll in public benefits, and coordinate with local resources to reduce hospital and ED utilization. #J-18808-Ljbffr The People Concern
