Outreach Case Manager: Drive Housing Solutions

PATH (People Assisting The Homeless)

PATH in Los Angeles, CA is hiring a Case Manager - Outreach to support individuals in ending homelessness. You will conduct intake, develop ISP, link participants to housing, income resources, and health services, and collaborate with partners in a fast‑paced outreach team. We value equity, professional growth, and a strong commitment to racial and social justice.
