Location: Casa Grande, Arizona
Job Type: Full-Time
Setting: Skilled Nursing Facility (SNF)

Position Overview

We are seeking a compassionate and motivated Physical Therapist (PT) to join a well-established Skilled Nursing Facility in Casa Grande, Arizona. This is an excellent opportunity to work in a collaborative rehabilitation environment focused on helping patients improve mobility, regain independence, and enhance their overall quality of life.

Both experienced Physical Therapists and new graduates are encouraged to apply.

Key Responsibilities

• Perform comprehensive patient evaluations and develop individualized treatment plans.

• Provide skilled physical therapy services to help patients achieve functional goals and maximize independence.

• Conduct initial evaluations, reassessments, and discharge planning.

• Deliver one-on-one treatment sessions in accordance with established standards of care.

• Collaborate with physicians, nursing staff, and interdisciplinary team members to coordinate patient care.

• Maintain accurate and timely clinical documentation.

• Educate patients and caregivers regarding treatment plans, home exercise programs, and discharge recommendations.

• Ensure compliance with facility policies, state regulations, and professional standards.

Schedule

• Full-Time Position

• Day Shift (8-hour shifts)

• Weekend flexibility may be required based on scheduling needs

• No on-call responsibilities

Patient Population

• Young Adult through Geriatric patients

QualificationsRequired

• Graduate of an accredited Physical Therapy program

• Current Arizona Physical Therapist License

• Current BLS Certification

• Arizona Fingerprint Clearance Card

Preferred

• Previous Skilled Nursing Facility (SNF) experience
