RN Case Manager

Job Summary:

We seek a highly skilled and experienced Registered Nurse with a minimum of 2 years of acute care experience to serve as a Case Manager. The role involves providing skilled nursing care, patient assessments, medication administration, treatment planning, and collaborating with the interdisciplinary team. Strong communication and collaboration with patients and healthcare providers are essential. This position requires flexibility to float between units as needed and includes weekend work every other weekend. Responsibilities: Required Skills & Certifications:

• Active Registered Nurse license in the state of practice.
• Minimum of 2 years acute care nursing experience.
• Strong clinical assessment, medication administration, and treatment planning skills.
• Proficiency in Epic electronic medical records system.
• Excellent communication and interpersonal skills.

Preferred Skills & Certifications:

• Experience with case management.
• Ability to work in fast-paced hospital environments.

Special Considerations:

• Required to work every other weekend.
• Call is not required for this position.
• Floating between units may be necessary based on facility needs.

Scheduling:

• Work schedule includes every other weekend shifts.
• Shift assignments may vary according to unit needs and staffing.
